How to Use Microsoft Forms 2021.
The surge in teleworking has forced many employers to switch to cloud-based software solutions. This means more people than ever have access to the suite of Microsoft Office 365 applications, including Microsoft Forms. Microsoft Forms is an online application that you can use to create quizzes, surveys, and other forms.
If you don’t have an Office 365 premium account, you can still use Microsoft Forms by signing up for a free Microsoft account. Below, we’ll explain how to use Microsoft Forms, including how to create a form, collect responses, and access results.
How to Create a Microsoft Form
Start by logging into form.office.com. From your dashboard, you can create a form or quiz, or use one of the free or additional templates provided by Microsoft.
Start by creating a new form and giving it a name and description. Add an image to the header of the form if you want.
Question types
Click the Add button and select a question type. Microsoft Forms offers many types of questions. Note. You can copy, delete, move, add subtitles, or require an answer to any question.
Choice. Use this question type for standard multiple-choice questions. Options include shuffling responses, adding an image or video, and displaying responses as a dropdown, radio buttons, or checkboxes for multiple responses.
Text. Use this type for questions that require a written response. Options include allowing long answers and adding an image or video.
Rating. In this type of question, respondents are asked to rate on a scale. Options include adding an image or video. The scale can range from two to ten levels, and you can choose between a numeric scale or an asterisk.
The date. Use this type of question if the answer is a date.
Rating. The ranking question type allows respondents to rank the options you provide by dragging and dropping them. You can add an image or video and up to ten options.
Likert. This type of question is similar to the rating. Likert’s question asks respondents to indicate how much they agree or disagree with a statement.
File downloaded. This type of question is only available to users with an OneDrive for Business account. Selecting this question type will create a new folder in your OneDrive for Business, and respondents will upload their files to that folder.
Net Promoter Score®. Microsoft says this question type is also similar to Rating questions, “pre-configured according to NPS standards, but you can customize them if you have specific requirements. If you use this type of question, you will have access to a summary page that shows the overall scores and distribution. ”
Add a new section
Select this if you want to create sections in your form. You can add, delete and move sections in the same way as questions.
At any time, you can see how your form will look to respondents by clicking on “Preview”.
Branching
Microsoft Forms also allows you to apply question logic (they call this “branching”). Click the More Options icon and select Add Branching. Navigate to the question you want to branch to and for each possible answer you can specify which question the respondent will be directed to next.
How to make your Microsoft Form attractive
Select “Theme” to see several options to improve the appearance of your form.
You can:
- choose one of Microsoft’s suggested theme ideas;
- choose a base color for form elements;
- find or download a background image of your choice
If you choose the third option, you can search for an image on Bing or upload a custom image from your OneDrive account or from your computer. You can also restrict image search results to only those marked with a Creative Commons license, which is really handy. Click the Add button to set the image you have chosen as the background image for your form.
Remember, you can always select Preview to see how your form will look to respondents on desktop or mobile.
Submitting the form and receiving responses
Click the Submit button to prepare the form for distribution. From there, you can copy the link to the form, generate a QR code, copy the inline code, share the link to the form by email, or post to Facebook or Twitter.

You can also create a link to share your form with someone else as a template.
To print the form, select the More Options icon and Print Form.
View responses
View the Answers tab to view the Answers panel. Microsoft gives you some basic data, such as how many responses you have, the average time it took respondents to fill out a form, and whether the form is active or closed.
If you select the More Options icon, you can delete all responses, print the summary, or create a link to the summary to share with others.
Click the View Results button to view each individual answer. Or select “Open in Excel” to export the form responses to a spreadsheet. The Answers panel also summarizes the answers to each question, often displaying the results in a chart.
